पापो विलग्नाधिपतिर्विलग्ने चन्द्रेण युक्तो यदि बालकः स्यात् ।
तदातिरोगं स हि केन्द्रसंस्थस्त्रिकोणलाभेषु गदं निहन्ति ॥ २४ ॥

यदि लग्नेश पापग्रह चन्द्रमा से युक्त होकर लग्न में हो तो बालक अत्यंत रोगी होता है। यदि वह केन्द्र-त्रिकोण और लाभभाव में हो तो रोग का नाश करता है ।। २४ ।।
If the lord of the ascendant is a natural malefic and is conjoined with the Moon in the ascendant, the native becomes extremely diseased or suffers from chronic ill health from childhood. However, if this conjunction of the malefic ascendant lord and the Moon is situated in a Kendra (1st, 4th, 7th, 10th), a Trikona (1st, 5th, 9th), or the 11th house, it destroys disease and grants good health.
Medical Astrology Commentary
This verse highlights the delicate relationship between the Ascendant (body), its lord (constitutional vitality), and the Moon (mind, fluids, growth, and immunity).
At first glance, the verse appears contradictory because the Ascendant itself is both a Kendra and a Trikona, yet the first half declares illness when the conjunction occurs in the Ascendant. The intended meaning is understood as follows:
• When a natural malefic becomes the Ascendant lord (as for Aries, Scorpio, Capricorn, or Aquarius ascendants) and joins the Moon in the Ascendant, the Moon becomes afflicted. Since the Ascendant directly governs the physical body and the Moon governs nourishment, vitality, and resistance, their affliction at the very seat of the body produces a weak constitution, recurrent illnesses, or chronic disease, particularly during childhood.
• When the same conjunction occurs in other Kendras (4th, 7th, or 10th), Trikonas (5th or 9th), or the 11th house, the placement acquires positional strength. The Ascendant lord remains strong by occupying auspicious houses, enabling it to overcome disease rather than produce it. The 11th house, being the 6th from the 6th, signifies victory over illness and restoration of health.
The conjunction should also be judged in light of planetary dignity and strength. A well-placed Ascendant lord in own sign, exaltation, or mūlatrikoṇa, receiving benefic aspects, can significantly reduce the adverse effects. Conversely, debility, combustion, affliction by additional malefics, or association with the 6th, 8th, or 12th lords can intensify the health concerns.
The Moon's condition is particularly important. An afflicted Moon weakens both the mind and the body's adaptive capacity, whereas a bright and well-supported Moon improves recovery and resilience even when associated with a natural malefic.
Finally, the verse should not be interpreted in isolation. The actual health outcome depends upon the combined strength of the Ascendant, its lord, the Moon, the 6th house, the 8th house (chronicity), the 12th house (hospitalisation and loss of vitality), and the relevant planetary periods (daśā and antar-daśā).
